VIS (Vaccine Information Statements)

Vaccine Information Statements (VIS) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine.

The vaccines we administer at Blue Fish are DTaP, Hepatitis A and B, Hib, HPV, MMR, Meningococcal, Pneumococcal Conjugate, Pneumococcal Polysaccharide, Polio, Rotavirus, Smallpox, Td, Tdap, and Chickenpox. During flu season, we offer both Live, Intranasal Influenza and Inactivated Influenza vaccines.
